Mapping Preventive Care Access at the State Level: A Nationwide Scan of Coverage Mandates

Federal and state statutes and regulations work to ensure access to preventive services designed to detect and prevent many conditions, including HIV, viral hepatitis, STDs, and tuberculosis. At the federal level, the Affordable Care Act requires most private health insurance plans and all Medicaid expansion programs to cover certain preventive services without cost-sharing, meaning that plans cannot require members to pay copayments, coinsurance, or deductibles for these services. This requirement applies to services with a grade A or B from the U.S. Preventive Services Task Force (USPSTF), services recommended by the Advisory Committee for Immunization Practices (ACIP) and adopted by the Centers for Disease Control and Prevention, additional preventive health services recommended for women by the Health Resources and Services Administration (HRSA), and preventive services for children and youth recommended by HRSA.

Many states also have laws and regulations that require insurers to cover these and other preventive services, reinforcing access to preventive care. These laws and regulations apply to individuals enrolled in state-regulated health coverage, including large-group fully-insured plans, individual and small-group plans, and Medicaid plans. The maps and table below catalog all state statutes and regulations (as well as those of the District of Columbia) in effect as of April 3, 2026, that require coverage of one or more of the following categories of preventive services:

  • Services with a grade A or B from the USPSTF
  • Services recommended by ACIP
  • Services recommended by HRSA for women
  • Services recommended by HRSA for children and youth
  • PrEP for HIV prevention
  • Prenatal or perinatal screenings for infectious diseases
  • Other guidelines from non-governmental bodies

Additionally, the “Cost-Sharing Prohibited” map and table show states prohibiting cost-sharing for certain preventative services.

Details about the statutes and regulations in each state, including the plans impacted and other limitations, are available by clicking on individual state names in the maps or the table. These statutes and regulations are subject to change through state legislative or regulatory processes, and any changes since April 3, 2026, are not reflected.
